Healthcare on Sint Eustatius: insured without premium, and 2,500 medical referrals a year
On Sint Eustatius you are insured as a resident by operation of law, without premium and without excess. But the island is small: what the care on Statia cannot do goes as a medical referral to Sint Maarten, and after that to Bonaire, Curaçao or Aruba, Colombia and the Netherlands, with flight, accommodation and daily allowance in the package. In 2025 that was 2,501 referrals from an island of 3,200 people. The flip side: a dental package that is almost empty from the age of eighteen, and a transition gap you insure yourself.
The healthcare system of Sint Eustatius is as friendly as Bonaire's and even more limited. Yes, you are insured as soon as you live lawfully on the island; it costs you no premium bill and no excess; and the trap lies in the weeks before your registration, in the dentist, and in the simple fact that an island of 3,200 residents has no hospital of Dutch size. Care and Youth Caribbean Netherlands processed 2,501 medical referrals from Statia in 2025, more than from Bonaire with its much larger population.
Insured without a policy
Whoever lives or works lawfully on Bonaire, Sint Eustatius or Saba is insured through Care and Youth Caribbean Netherlands (ZJCN): no policy to take out, no nominal premium, no excess. The cover is broad, from GP to long-term care, and your Dutch basic insurance really does end with it: whoever is still subject to compulsory insurance in the Netherlands is not eligible, and the insurance office asks on registration for proof of the end date of your Dutch policy. A CAK construction with S1 forms does not exist here; pensioners are simply insured as residents.
When can you register, and where?
Only after you are in the population register of Sint Eustatius. Then you go to the ZJCN office in the Mazinga building in Oranjestad, open Monday to Thursday and on Friday by appointment only, with your ID card, your declaration by operation of law or proof that the renewal is pending, an employer's statement if you work for a local employer, and the proof of the end date of your Dutch insurance. Whoever has an employment contract of more than three months with a local employer can register on that employer's statement for the duration of the contract. After that you choose a GP; you can switch twice a year, on 1 April and 1 October.
Between arrival and that registration you are uninsured. ZJCN itself writes that you bridge that period with your own health insurance; arrange it before departure, not after.
What can the island do, and what not?
ZJCN's referral route is different for Statia than for Bonaire, and that difference shapes your life here. If a treatment is not available on the island, you are first referred to Sint Maarten; if it is not available there, then successively to Bonaire, to Curaçao or Aruba, to Colombia and finally to the European Netherlands. The insurance office is honest about what that means: from the Windward Islands a referral usually goes by small aircraft, on fixed days and with a limit on luggage, and specialists are not present all year, so that an insured person sometimes meets a different doctor every three months. The figures say the rest: from 1,297 referrals in 2020 to 2,501 in 2025, radiology and ophthalmology in front.

How does the referral work in practice?
What the island cannot do, the insurance arranges with a medical referral, and the special thing is what is then reimbursed: not only the treatment, but also the flight, the airport taxes, the transport to and from the airport, the hotel or an allowance for staying with family, and a daily allowance for you and your companion if you have one. You get a ZJCN case manager as a fixed point of contact. A companion may come along on indication, for instance for a child, with limited mobility or for childbirth; a minor can under conditions get two companions. The condition is as simple as it is hard: ZJCN must give permission in advance, except in an emergency. Whoever arranges a treatment in the Netherlands on their own initiative pays for it themselves.
Plan around the flight, not around the appointment. A referral from Statia starts with a flight to Sint Maarten in a small aircraft on fixed days, with limited luggage. Whoever works arranges leave for more than the treatment day, and whoever needs a companion asks for it at the referral straight away.
The flip side: the dentist and the diary
The limits are in two places. The dentist: up to eighteen the cover is complete, after that only dentures, medically necessary treatments and surgical care remain, and implants, simple extractions and gum surgery are not reimbursed. So plan the big dental check before you emigrate. And the diary: your ID card is valid for five years on Statia and as an insured person you must always carry it, so that the care provider can check that you are registered. The schools on Sint Eustatius come under the Dutch Inspectorate of Education, but the education is in English: four small primary schools and one secondary school, the Gwendoline van Putten School, which has run the Caribbean CXC examination system since 2015 instead of havo, vwo or mbo. Buying on Sint Eustatius goes the way it should in the Caribbean Netherlands: through the notary, with 5% transfer tax and a Land Registry that has come under the Dutch Kadaster since 2021, with an office at Fort Oranje 5. After that one question counts: do you live there yourself? The owner-occupied home is exempt from property tax; a second home pays 0.7%, without the surcharges Bonaire does levy. Your driving licence on Sint Eustatius: recognised, valid for five years and a driving test if you wait too long
Your Dutch driving licence is recognised as equivalent to the island licence on Sint Eustatius: no exchange is needed as long as it is valid. If it expires you have two routes: a Statia licence at the Census Office for $ 37, valid for five years, with a health certificate from a doctor on the island, or renewal at the RDW from abroad for € 36.40. The trap is in article 106 of the traffic ordinance: whoever lets a licence lapse for more than two years must prove driving competence again.
