Healthcare on Bonaire: one hospital, and the plane tickets are in the package

On Bonaire you are insured by operation of law as a resident — without a premium, without an excess. What the island can't do, the insurance arranges with medical referrals abroad to Curaçao, Colombia or the Netherlands, including tickets and accommodation. The flip side: a dental package that is virtually empty from the age of eighteen, and a transition gap you have to insure yourself.

The Bonairean healthcare system is the friendliest of all destinations on this site — and at the same time the most bounded. Both sides deserve the whole story.

Insured without a policy

Whoever lawfully lives or works on Bonaire is insured by operation of law through Zorg en Jeugd Caribisch Nederland (ZJCN): no policy to take out, no nominal premium, no excess. The funding sits in the wage tax, and the cover is broad — GP, hospital, medicines, physiotherapy, midwifery, long-term care. Your Dutch basic insurance thereby really does end too: the health insurance office even demands on registration proof of the end date of your Dutch policy. And unlike with the European pension countries there is no CAK construction with S1 forms and country-of-residence factors — pensioners are simply insured there as residents, like everyone else.

One gap requires your own attention: registration is only possible once your residence is settled (sédula or IND declaration), and between arrival and that moment you are uninsured. The health insurance office itself advises bridging those weeks with a temporary insurance — arrange it before departure, not after.

One hospital, and the island's limit

Fundashon Mariadal in Kralendijk is the only hospital: some eighty beds, accident and emergency, maternity, dialysis, and for a handful of specialisms visiting consultants flown in from the Netherlands and Curaçao. That is a lot for an island of twenty thousand inhabitants — and it stops somewhere. Complex oncology, cardiac surgery, neurosurgery: Bonaire can't do that, and it doesn't need to be able to.

The referral abroad: the package with wings

What the island can't do, the insurance arranges with a medical referral abroad (medische uitzending): a referral to Curaçao, Aruba, Sint Maarten, Colombia or the Netherlands — "as close by as possible, further only if necessary". The special thing is what is reimbursed: not only the treatment, but also the plane tickets, the accommodation and a daily allowance, for the patient and — where indicated — a companion. Children, the seriously ill and births with risk get a companion as standard. The condition is as simple as it is hard: permission in advance, except in acute emergencies. Whoever arranges a treatment in the Netherlands on their own initiative pays for it themselves.

The flip side: the dentist and the diary

The limitations sit in two places. The dentist: up to the age of eighteen the cover is complete, after that only dentures and medically necessary procedures remain — check-ups, fillings and crowns are at your own expense. The practical conclusion: plan the big dental overhaul before your emigration. And the diary: an expired sédula also blocks your access to care at the counter, so the renewal date of that card is a health matter on Bonaire.
