Healthcare in Suriname: the SZF for the basics, you yourself for the rest
Residents have a legal right to the basic care package and can turn to the State Health Insurance Fund — but the package is limited, the hospitals struggle with shortages, and for serious cases Suriname turns abroad. There is no CAK route: you arrange the evacuation cover privately, before departure.
The greatest advantage of Surinamese healthcare is at once its pitfall: everything sounds familiar. Dutch-speaking doctors, a health insurance fund, a basic package — you get used to it so quickly that people forget the system underneath works fundamentally differently from the Dutch Zvw. This article explains what it covers, what it doesn't, and what you arrange before departure.
No CAK route — the Zvw really stops
Suriname is not on the list of treaty countries of the Dutch foreign-country scheme: the CAK route doesn't exist, your basic insurance ends on the day of deregistration, and no treaty contribution or treaty cover takes its place. What you build up, you build up locally and privately from here.
The SZF: the basics, for residents
The National Basic Health Insurance Act gives all residents — including Dutch citizens living in Suriname — the right to the care in the basic package, and the State Health Insurance Fund (SZF) is the largest administrator.
Here two premium tables get mixed up, and that difference is worth hundreds of euros. The statutory minimum premiums of the basic health insurance (SRD 55 to SRD 240 a month, rising with age) are the premiums the law is about — with a government exemption for Surinamese nationals under seventeen and over sixty. But the route a Dutch newcomer without an employer walks in practice is the SZF's voluntary personal insurance, and that has its own table:

For someone over 61 that is around € 38 a month — still low by Dutch standards, but almost seven times the amount that gets passed around. Advance payment earns a discount: 5% from six months, 7% for a whole year.
Two hurdles you want to know about before departure. A medical examination is mandatory — a form from your GP plus an interview at the Medical Service — and the premium is only set after that examination, so a surcharge is possible. And whoever registers without a valid residence permit pays twelve months' premium in advance: for someone over 61 around € 460 at once. Whoever is of Surinamese origin and has permission for a short stay of at least six months is exempt from that advance payment — one of the few places where the PSA route also saves money.
Know what you are buying: the basic package covers the GP, medicines from the complaints package and hospital care at the contracted institutions — and it stops where the private clinics, the extensive diagnostics and abroad begin.
The limits: shortages, medicines and the evacuation question
The honest picture, also according to the Dutch travel advice: care is not of a good standard everywhere, hospitals struggle with shortages of people and resources, and the availability of medicines varies. The Academic Hospital Paramaribo is being modernised and the Sint Vincentius is the best-known private alternative — but for complex or serious cases Suriname turns abroad, by air ambulance to Colombia, Trinidad or the Netherlands.
That is why the core rule of this corridor is: an international policy with evacuation and repatriation cover is not a luxury but the real safety net — arranged before departure, with the acceptance question asked before every irreversible step. The SZF and that policy side by side is the normal pattern: the basics local, the risk international.
Practically alongside that: take a generous supply of chronic medication in the original packaging (with a medicine declaration where needed), and cover the gap between your Dutch end date and your first insured day with a bridging policy that carries South America.
The order
First the acceptance question at an international insurer, then the bridging cover, only then deregister. After arrival: first the registration as a resident at the Central Bureau for Civil Affairs (Centraal Bureau voor Burgerzaken) — without that registration no SZF registration — and then both covers active. Whoever keeps to this order is nowhere uninsured along the way; whoever reverses it is, for months.
